In order to help us better care for your cats, the more information we have, the better equipped we will be.These questionnaires, when completed properly, can save the life of a cat. Please take the necessary time to complete the questionnaire appropriately.

 

Hotel Questionnaire

The following questionnaire will be e-mailed to all clients. We will require a completed questionnaire for each client which we will keep permanently on-file. If you wish to use the Questionnaire provided below, please highlight the complete text, copy it (Ctrl C) and then paste it into a Word document. You can then e-mail it directly to us.

Cat profile - please answer these questions for each cat.

  2. Age? Date of Birth (if you know it)?
  3. If over 10 years old, when was your cats last Geriatric Examination?
  4. Breed? Colour?
  5. Does the cat go outdoors?
  6. Is it a male or female and is it spayed or neutered?
  7. When was the last medical exam? Are shots up to date?
  8. Any medications? Any surgeries?
  9. Any treatments? Any medical conditions?
  10. Any supplements?
  11. Are there any behaviors, illnesses, little quirks that I should watch for?
  12. Has your cat ever displayed aggressive or nasty behavior toward strangers?
  13. How does your cat have fun? (ie: toys, beating up her sister, etc.)
  14. Does your cat like to be brushed or combed?
  15. Does your cat hide, and, if so, where?
  16. Where does your cat usually sleep?
  17. Are there any areas on the cat's body that it doesn't want to have touched?
  18. Are there special favourite places where the cat wants to be stroked, or scratched?
  19. Does your cat have any fears or dislikes?
  20. How often does your cat eat?
  21. What kind of food? Wet? Dry? What brands?
  22. How much food? Do you measure it? Do you keep bowls full at all times?
  23. If you are having two visits a day from me, will the cat have the same meal each time?
  24. Does the cat drink tap water? Or Filtered?
  25. Is the cat allowed to have treats? What kind of treats?
  26. Is the cat allowed to have catnip? Plain catnip, or just the kind inside toys?
  27. Describe your cat's personality, (some hints: friendly, shy, nervous, affectionate etc.)
  1. Do you have a good photo of your cat(s) that I may put into my client database? Are you able to email the picture to me? May I take a photo of your cat when I am there, in case you don't have a photo that you can send me? I have a lot of cats to keep track of and having pictures helps. These photos are used for this purpose only and not used in any other way.

Emergency Information ( this information is required )

I will need a telephone number where I can reach you while you are away. Please either leave it for me when you depart, or send it to me with this questionnaire. Each time you go away and book my services I will need to have an emergency number.
